Evergreen Medical Center in Evergreen was recently selected to receive services through the Rural Hospital Performance Improvement Project.

One of these activities includes the distribution of a community questionnaire to solicit feedback from area residents to help the hospital improve and strengthen local health services and programs.

Through this process the hospital hopes to increase community awareness of these resources and identify local need.

The Rural Hospital Performance Improvement Project will randomly select 1,000 area residents who receive a written questionnaire and a postage paid reply envelope.

These selected residents will be asked what they think about various aspects of their local health care delivery system and what changes they would like to see.

These questionnaires will be distributed in Evergreen, Andalusia, Brewton, Greenville, and Monroeville during the month of October. Residents are urged to complete and return the forms. Their input is essential.

Information and data gathered through these community questionnaires will be summarized to ensure confidentiality and anonymity and will be shared with Evergreen Medical Center board and administration.

This information will help the facility develop a strategic plan for the future to meet the community’s needs.
